Textbook Rating Page

Several of you have asked for a textbook rating form. I will collect the input from these forms, and post them on the CRISP server. Please fill in all the fields. If several of you wish to rate the same text, I will compile all the ratings into one page, giving the average rating, as well as the high and low ratings. I will also cat all the comments so you can see everybody's comments on each text.
